Parci\u00e1lny tlak kysl\u00edka je v obr\u00e1tenom pomere k nadmorskej v\u00fd\u0161ke, teda, alveol\u00e1rny parci\u00e1lny tlak kles\u00e1 so vzrastaj\u00facou nadmorskou v\u00fd\u0161kou, \u010do vedie k tomu, \u017ee pre pacientov je vy\u0161\u0161ie riziko v\u00e1\u017enej desatur\u00e1cie alebo hypox\u00e9mie. U pacientov s v\u00e1\u017enej\u0161\u00edm p\u013e\u00facnym ochoren\u00edm m\u00f4\u017ee cestovanie lietadlom alebo pobyt vo ve\u013ekej nadmorskej v\u00fd\u0161ke vies\u0165 k hypoxii pod PaO2 6,6kPa (50 mmHg), \u010do sa pova\u017euje za v\u00fdznamn\u00fa hrani\u010dn\u00fa hodnotu.<\/p>\n<p>Komer\u010dn\u00e9 lietadl\u00e1 operuj\u00fa v r\u00f4znych kab\u00ednov\u00fdch v\u00fd\u0161kach a pri r\u00f4znych tlakoch v rozp\u00e4t\u00ed od 300 m do 3000m. Zdru\u017een\u00e9 leteck\u00e9 \u00farady (Joint Aviation Authority) obmedzili priemern\u00fa kab\u00ednov\u00fa v\u00fd\u0161ku na 2348 m. V d\u00f4sledku tohto kroku vdychovan\u00fd kysl\u00edk, ktor\u00fd je v kab\u00edne k dispoz\u00edcii, sa pohybuje v rozmedz\u00ed 14,7% po 16,8%, v z\u00e1vislosti od leteckej linky a lietadla (v porovnan\u00ed s 21% na hladine mora). Uk\u00e1zalo sa, \u017ee \u013eudia s CF m\u00f4\u017eu tolerova\u0165 hodnoty PaO2 pod 6,6kPa (50 mm Hg) nieko\u013eko hod\u00edn bez srdcovej dekompenz\u00e1cie alebo cerebr\u00e1lnych sympt\u00f3mov, \u010do nazna\u010duje, \u017ee smernice BTS nemusia by\u0165 pre t\u00fdchto pacientov \u00faplne smerodajn\u00e9. Mo\u017eno predpoklada\u0165, \u017ee pacienti s CF s\u00fa adaptovan\u00ed na ob\u010dasn\u00fa \u0165a\u017ek\u00fa hypoxiu, najm\u00e4 po\u010das cvi\u010denia. Stoj\u00ed za zmienku, \u017ee jednotlivci s CF zvy\u010dajne nenes\u00fa \u017eiadne kardiovaskul\u00e1rne riziko a s\u00fa v priemere mlad\u0161\u00ed ako pacienti s chronickou ob\u0161truk\u010dnou p\u013e\u00facnou chorobou. Samozrejme, pova\u017eova\u0165 len v\u00fdlu\u010dne PaO2 za d\u00f4vod doplnkovej d\u00e1vky kysl\u00edka po\u010das letu je \u201eprive\u013ek\u00e9 zjednodu\u0161enie\u201c. V ide\u00e1lnom pr\u00edpade by mal bra\u0165 \u00favahy stupe\u0148 hyperventil\u00e1cie, ktor\u00fd bude pacient schopn\u00fd znies\u0165, ako aj jeho hodnotu hemoglob\u00ednu, v\u00fdkon srdca a \u010fal\u0161ie faktory, ktor\u00e9 spoluur\u010duj\u00fa dod\u00e1vanie kysl\u00edka do tkan\u00edv. Osobitn\u00e1 pozornos\u0165 by sa mala venova\u0165 pacientom so z\u00e1va\u017en\u00fdm bronchi\u00e1lnym kolapsom alebo ob\u0161trukciou, ke\u010f\u017ee nemusia by\u0165 schopn\u00ed kompenzova\u0165 hypoxiu hyperventil\u00e1ciou.\u00a0 Pacienti s cirh\u00f3zou pe\u010dene a s pa\u017eer\u00e1kov\u00fdmi k\u0155\u010dov\u00fdmi \u017eilami m\u00f4\u017eu ma\u0165 vy\u0161\u0161ie riziko krv\u00e1cania po\u010das pred\u013a\u017een\u00fdch letov alebo pri pobytoch vo ve\u013ekej nadmorskej v\u00fd\u0161ke.<\/p>\n<p>Absol\u00fatnymi kontraindik\u00e1ciami cestovania lietadlom, pokia\u013e nejde o zdravotn\u00fa dopravu pacienta za \u00fa\u010delom z\u00e1chrany \u017eivota, by mohli by\u0165: z\u00e1va\u017en\u00e1 respira\u010dn\u00e1 nedostato\u010dnos\u0165, re\u00e1lny infarkt alebo hemodynamick\u00e1 nestabilita. Pacienti, ktor\u00ed mali pneumotorax, by mali po\u010dka\u0165 6 t\u00fd\u017ed\u0148ov po definit\u00edvnom chirurgickom z\u00e1sahu.<\/p>\n<blockquote><p><strong>Pre pacientov s CF s\u00fa d\u013a\u017eka letu a zdravotn\u00fd stav hlavn\u00fdmi indik\u00e1tormi zdravotnej bezpe\u010dnosti po\u010das cesty lietadlom.\u00a0V\u0161etci chor\u00ed s CF by sa mali o cestovan\u00ed lietadlom porozpr\u00e1va\u0165 so svoj\u00edm odborn\u00fdm lek\u00e1rom.<\/strong><\/p><\/blockquote>\n<p>V\u00e4\u010d\u0161ina \u00fadajov o hypoxii, s\u00favisiacej s letom, sa nez\u00edskala zo \u0161t\u00fadi\u00ed o jednotlivcoch s CF, ale bola ur\u010den\u00e1 z in\u00fdch chronick\u00fdch ob\u0161truk\u010dn\u00fdch p\u013e\u00facnych chor\u00f4b. V minulosti bolo vypracovan\u00fdch a vyhodnoten\u00fdch viacero testov na meranie sp\u00f4sobilosti pacientov pre let. V s\u00fa\u010dasnosti sa na simulovanie podmienok za letu pou\u017e\u00edva umiestnenie pacienta do hypobarickej komory alebo hypoxick\u00fd test v n\u00edzkej nadmorskej v\u00fd\u0161ke. Testy by sa mali vykon\u00e1va\u0165 vtedy, ke\u010f je pacient stabilizovan\u00fd a m\u00e1 vycestova\u0165 v relat\u00edvne kr\u00e1tkom \u010dase (najnesk\u00f4r za tri mesiace).<\/p>\n<p>Pobyt v hypobarickej komore umo\u017e\u0148uje dobr\u00fa simul\u00e1ciu podmienok po\u010das letu; nie je v\u0161ak v\u017edy k dispoz\u00edcii, netoleruj\u00fa ho v\u0161etci pacienti a je drah\u00fd. \u201eAerospace Medical Association\u201c a Britsk\u00e1 hrudn\u00edkov\u00e1 spolo\u010dnos\u0165 odpor\u00fa\u010daj\u00fa inhala\u010dn\u00fd hypoxick\u00fd test (HIT), \u010do predstavuje d\u00fdchanie hypoxickej zmesi plynov (15% kysl\u00edka doplnen\u00e9ho dus\u00edkom )po\u010das 20 min\u00fat, s cie\u013eom predpoveda\u0165 hypox\u00e9miu pri maxim\u00e1lnom povolenom tlaku v kab\u00ednovej v\u00fd\u0161ke 2438 m (8000 st\u00f4p). Kysl\u00edkov\u00e1 terapia za letu sa odpor\u00fa\u010da vtedy, ke\u010f HIT PaO2 klesne pod 6,6-7,4 kPa (50-55 mmHg), (vi\u010f tabu\u013eku \u201eSimulovan\u00e9 v\u00fd\u0161ky r\u00f4znymi FiO2\u201c na <a href=\"http:\/\/ecorn-cf.eu\/index.php?id=265&amp;L=8\">http:\/\/ecorn-cf.eu\/index.php?id=265&amp;L=8<\/a>).<\/p>\n<p>Viacer\u00ed autori dok\u00e1zali, \u017ee hypobarick\u00e1 komora aj HIT s\u00fa bezpe\u010dn\u00e9 a predikuj\u00fa PaO2 pacienta s ob\u0161truk\u010dnou p\u013e\u00facnou chorobou v nadmorskej v\u00fd\u0161ke. Obidva testy sa v\u0161ak vykon\u00e1vaj\u00fa v stabilnom prostred\u00ed a nemusia \u00faplne reprezentova\u0165 fyzick\u00fd stres a variabilitu prostredia leteck\u00e9ho cestovania, najm\u00e4 za dlh\u00fdch letov. Ke\u010f sa po\u010das testu HIT pridalo 50m ch\u00f4dze, hypox\u00e9mia pacientov s respira\u010dn\u00fdmi chorobami sa v\u00fdrazne zhor\u0161ila. Na druhej strane v\u00e4\u010d\u0161ina pacientov, ktor\u00ed by z\u00edskali n\u00e1rok na dodanie kysl\u00edka po\u010das letu na z\u00e1klade HIT, cestovala v poriadku bez \u010fal\u0161ieho kysl\u00edka. Preto test HIT PaO2 neindikuje automaticky potrebu kysl\u00edkovej terapie za letu. Sk\u00f4r by mal vies\u0165 k informovanej diskusii medzi pacientom a lek\u00e1rom o v\u00fdhod\u00e1ch a rizik\u00e1ch lietania s\u00a0 doplnkov\u00fdm kysl\u00edkom alebo bez neho.<\/p>\n<p><strong>Faktory,\u00a0 ktor\u00e9 treba zv\u00e1\u017ei\u0165 pri odpor\u00fa\u010dan\u00ed doplnkov\u00e9ho kysl\u00edka:<\/strong><\/p>\n<ul>\n<li>Lek\u00e1rska anamn\u00e9za<\/li>\n<li>Stabilita p\u013e\u00facnej choroby<\/li>\n<li>Predch\u00e1dzaj\u00face sk\u00fasenosti (symptomatick\u00e9 na predch\u00e1dzaj\u00facich dovolenk\u00e1ch)<\/li>\n<li>Trvanie letu<\/li>\n<li>D\u013a\u017eka dovolenky<\/li>\n<li>Destin\u00e1cia, nadmorsk\u00e1 v\u00fd\u0161ka, aktivity,<\/li>\n<li>Lek\u00e1rske vy\u0161etrenie<\/li>\n<li>Testy fungovania p\u013e\u00fac<\/li>\n<li>Anal\u00fdza plynov v krvi<\/li>\n<li>Satur\u00e1cia kysl\u00edka v art\u00e9ri\u00e1ch<\/li>\n<li>V\u00fdsledky hypoxick\u00e9ho sk\u00fa\u0161obn\u00e9ho testu<\/li>\n<\/ul>\n<p>V poslednej dobe sa objavili spr\u00e1vy, \u017ee pacienti, ktor\u00ed mali opakovane\u00a0 FEV1 vy\u0161\u0161ie ako 55% predpovedali, \u017ee je ve\u013emi nepravdepodobn\u00e9, \u017ee bud\u00fa ma\u0165 v\u00fdznamn\u00fa desatur\u00e1ciu po\u010das cesty lietadlom. Ak je v\u0161ak sp\u00f4sobilos\u0165 pacientov pre let spochybnen\u00e1, vy\u0161\u0161ie pop\u00edsan\u00e9 testy by sa mali vykona\u0165. Ak tieto testy nie s\u00fa k dispoz\u00edcii (napr. z h\u013eadiska geografickej polohy alebo kv\u00f4li zdrojom), odpor\u00fa\u010da sa pacientom s mo\u017en\u00fdm rizikom desatur\u00e1cie predp\u00edsa\u0165 kysl\u00edk na doplnenie po\u010das letu.<\/p>\n<p>Je potrebn\u00fd \u010fal\u0161\u00ed v\u00fdskum na to, aby bolo mo\u017en\u00e9 ur\u010di\u0165, ako stupe\u0148 a d\u013a\u017eka trvania hypobarickej hypoxie ovplyvnia relat\u00edvne riziko nepriazniv\u00fdch n\u00e1sledkov u pacientov s CF.<\/p>\n<blockquote><p><strong>Predpove\u010f \u010di bude po\u010das letu potrebn\u00fd dodato\u010dn\u00fd kysl\u00edk m\u00f4\u017ee by\u0165 u\u017eito\u010dn\u00e1 najm\u00e4 pri pacientoch s CF, ktor\u00ed maj\u00fa v\u00e1\u017enu p\u013e\u00facnu chorobu, vyskytuj\u00fa sa u nich \u010fal\u0161ie ochorenia so stavmi zhor\u0161ovan\u00fdmi hypox\u00e9miou, maj\u00fa vopred existuj\u00facu po\u017eiadavku na kysl\u00edk alebo v minulosti nezn\u00e1\u0161ali cestovanie lietadlom.<\/strong><\/p><\/blockquote>\n<p>Leteck\u00e9 spolo\u010dnosti zvy\u010dajne vy\u017eaduj\u00fa, aby pacienti nosili predpis na kysl\u00edk v\u017edy so sebou. Mnoh\u00e9 spolo\u010dnosti maj\u00fa svoj vlastn\u00fd zdravotn\u00fd formul\u00e1r \u0161pecifick\u00fd pre dan\u00fa spolo\u010dnos\u0165, ktor\u00fd mus\u00ed vyplni\u0165 lek\u00e1r. Formul\u00e1re oby\u010dajne platia jeden rok a mali by obsahova\u0165 inform\u00e1cie o sp\u00f4sobilosti cestuj\u00faceho cestova\u0165 lietadlom a o jeho\/jej schopnosti obsluhova\u0165 kysl\u00edkov\u00fd pr\u00edstroj a primerane reagova\u0165 na jeho sign\u00e1ly. Mali by sa \u0161pecifikova\u0165 f\u00e1zy letu, kedy je kysl\u00edk medic\u00ednsky potrebn\u00fd (rolovanie, \u0161tart, let a prist\u00e1vanie). Ke\u010f\u017ee ve\u013ea dopravcov \u00fa\u010dtuje za doplnkov\u00fd kysl\u00edk poplatok, je treba vopred kontaktova\u0165 poskytovate\u013ea poistenia, aby sa ur\u010dila v\u00fd\u0161ka \u00fahrady za kysl\u00edk doplnen\u00fd po\u010das cesty. Okrem toho pacient mus\u00ed kontaktova\u0165 lok\u00e1lnych poskytovate\u013eov pou\u017e\u00edvania kysl\u00edka po\u010das ka\u017ed\u00e9ho medziprist\u00e1tia a v kone\u010dnej destin\u00e1cii. V s\u00falade s antidiskrimina\u010dnou legislat\u00edvou maj\u00fa leteck\u00e9 spolo\u010dnosti a cestovn\u00e9 kancel\u00e1rie zak\u00e1zan\u00e9 odmietnu\u0165 cestuj\u00facim slu\u017ebu na z\u00e1klade zn\u00ed\u017eenej mobility, pokia\u013e nejde o \u201eriadne zd\u00f4vodnen\u00e9\u201c odmietnutie z bezpe\u010dnostn\u00fdch d\u00f4vodov. Preto od r. 2008 maj\u00fa zdravotne postihnut\u00ed \u013eudia pr\u00e1vo na poskytnutie bezplatnej pomoci na v\u0161etk\u00fdch eur\u00f3pskych letisk\u00e1ch, ako aj na palube lietadla, ktor\u00e9 odlieta z E\u00da. Cestuj\u00faci by si mali pred odletom zisti\u0165 u svojej leteckej spolo\u010dnosti, ktor\u00e9 slu\u017eby s\u00fa zdarma. Zoznam povinnost\u00ed let\u00edsk a leteck\u00fdch spolo\u010dnost\u00ed, na ktor\u00e9 musia tieto nies\u0165 n\u00e1klady, zah\u0155\u0148a poskytnutie pr\u00edslu\u0161nej infra\u0161trukt\u00fary pre zdravotne postihnut\u00fdch cestuj\u00facich, ako aj dopravu pre kolieskov\u00e9 kresl\u00e1 a potrebn\u00e9 zdravotn\u00edcke vybavenie. Niektor\u00e9 spolo\u010dnosti pon\u00fakaj\u00fa \u0161peci\u00e1lne pozemn\u00e9 slu\u017eby, ako napr. osobitn\u00fa odbavovaciu prek\u00e1\u017eku, prednostn\u00e9 nast\u00fapenie do lietadla at\u010f. Cestuj\u00faci s obmedzenou mobilitou by sa mali obr\u00e1ti\u0165 na leteck\u00fa spolo\u010dnos\u0165 minim\u00e1lne 48 hod\u00edn pred odletom kv\u00f4li potrebnej pomoci, ktor\u00fa pre nich potom person\u00e1l letiska zariadi.<\/p>\n<p>Pre cestuj\u00facich, ktor\u00ed potrebuj\u00fa po\u010das letu doplnkov\u00fd kysl\u00edk m\u00e1 ka\u017ed\u00e1 leteck\u00e1 spolo\u010dnos\u0165 vlastn\u00fd postup pre dopravu kysl\u00edka na palubu a pou\u017eitie kysl\u00edka za letu (podrobn\u00e9 inform\u00e1cie n\u00e1jdete na <a href=\"http:\/\/www.european-lung-foundation.org\/\">http:\/\/www.european-lung-foundation.org\/<\/a>). Odpor\u00fa\u010da sa informova\u0165 leteck\u00fa spolo\u010dnos\u0165 pri rezervovan\u00ed letu a znova 48 hod\u00edn pred letom o tom, \u017ee pacient pl\u00e1nuje cestova\u0165 s kysl\u00edkom.<br \/>\nAk je potrebn\u00e9 pou\u017ei\u0165 lie\u010debn\u00fd kysl\u00edk po\u010das letu, existuje nieko\u013eko mo\u017enost\u00ed. Pod\u013ea medzin\u00e1rodn\u00e9ho pr\u00e1va nie je dovolen\u00e9 ma\u0165 na palube lietadla tekut\u00fd kysl\u00edk. V\u00e4\u010d\u0161ina eur\u00f3pskych leteck\u00fdch spolo\u010dnost\u00ed dovol\u00ed cestuj\u00facim ma\u0165 so sebou a pou\u017ei\u0165 mal\u00fa f\u013ea\u0161u s plynn\u00fdm kysl\u00edkom, ktorej objem neprekro\u010d\u00ed 2 l (UN-1072, 200 barov). T\u00e1to slu\u017eba je v\u0161ak zak\u00e1zan\u00e1 pri letoch do USA, Kanady a Mexika, pri letoch z t\u00fdchto \u0161t\u00e1tov a cez tieto \u0161t\u00e1ty.\u00a0 Kysl\u00edkov\u00e1 f\u013ea\u0161a mus\u00ed by\u0165 po\u010das letu ulo\u017een\u00e1 v pevnom obale pod predn\u00fdm sedadlom, pod\u013ea mo\u017enosti aj po\u010das pou\u017e\u00edvania. Niektor\u00e9 leteck\u00e9 spolo\u010dnosti dovo\u013euj\u00fa ulo\u017ei\u0165 pr\u00e1zdne kysl\u00edkov\u00e9 vybavenie do bato\u017einy, mus\u00ed sa v\u0161ak skontrolova\u0165, \u010di je pr\u00e1zdne a regul\u00e1tor sa mus\u00ed odstr\u00e1ni\u0165. Je d\u00f4le\u017eit\u00e9 vopred si u leteckej spolo\u010dnosti overi\u0165, \u010di dovolia ma\u0165 so sebou pr\u00e1zdne syst\u00e9my\/kanistre ako ulo\u017een\u00fa alebo skontrolovan\u00fa bato\u017einu.<\/p>\n<p>V\u00e4\u010d\u0161ina leteck\u00fdch spolo\u010dnost\u00ed za \u010fal\u0161\u00ed poplatok poskytuje kysl\u00edk na palube v z\u00e1vislosti na trase (kontinent\u00e1lne alebo medzikontinent\u00e1lne lety). Pre ka\u017ed\u00fd let zvy\u010dajne plat\u00ed zvl\u00e1\u0161tny poplatok. Sp\u00f4sob podania kysl\u00edka (kontinu\u00e1lne, na po\u017eiadanie) a maxim\u00e1lny prietok sa medzi jednotliv\u00fdmi spolo\u010dnos\u0165ami l\u00ed\u0161ia a m\u00f4\u017eu by\u0165 obmedzen\u00e9. Najnov\u0161\u00edm prenosn\u00fdm zariaden\u00edm, ktor\u00e9 je pre pou\u017e\u00edvate\u013eov kysl\u00edka k dispoz\u00edcii, s\u00fa prenosn\u00e9 kysl\u00edkov\u00e9 koncentr\u00e1tory (POC). S\u00fa \u013eahk\u00e9 a m\u00f4\u017eu by\u0165 napojen\u00e9 na sie\u0165 elektrickej energie alebo na bat\u00e9riu. Od m\u00e1ja 2009 s\u00fa POC povolen\u00e9 na v\u0161etk\u00fdch letoch za\u010d\u00ednaj\u00facich alebo kon\u010diacich v Spojen\u00fdch \u0161t\u00e1toch. Ministerstvo dopravy schv\u00e1lilo limitovan\u00fd po\u010det POC na pou\u017eitie po\u010das cestovania lietadlom (9\/2009: Inogen One, AirSepLifestyle, AirSep Freestyle, SeQual Eclipse and Respironics EverGO).
